Fees are assigned on case basis and will be agreed upon prior to professional consultation.
Consideration is given to the acuity of patient's condition, patient resources and format of consultation.

$75 phone call/text/email consultation or virtual visit for new patients 
(Including referrals, prescriptions, informational advice)

Professional Fees for medical consultation may be eligible for insurance reimbursement 
based on patient's contracted out-of-network health insurance rates 
Professional services are eligible for HSA (Health Savings Account) expenses 

Fees are collected at time of service via PayPal which accepts all major credit cards through secure network.
Receipts for insurance reimbursement (if applicable) will be available upon patient request.
Records will be securely stored and only shared with designated patient permission.
